e. Bronchiectasis and Pneumothorax.

f. Pneumonectomy.

g. Acute Mycotic Diseases, including, but not limited to, Coccidioidomycosis and Histoplasmosis.

h. Acute Pleurisy.

i. Malignant Diseases.
