a. Uncontrolled Thyroid Disease.

b. Diabetes Mellitus.

c. Adrenal Dysfunction, including but not limited to, Addison's Disease and

Cushing's Disease.

d. Symptomatic Hypoglycemia.

e. Untreated Thyroid Malignancy.
